Support Coordination
Support Coordination at Empowered Care Services assists participants in understanding NDIS plans.
Our role is to make the NDIS plan as straight forward as possible for participants and families. Our priority is to gain understanding of you and your NDIS goals. We will also assist in implementing plan reviews and access services from a range of different providers.

Receive NDIS Plan

Arrange a meeting with the relevant stakeholders to review the NDIS plan and goals

Discussing relevant services that are applicable to the participants NDIS goals

Complete a progress report within 3 months of the plan

Final plan review
